Hospital Transport and Post Discharge support
As part of a contract with the NHS, we are providing transport home from hospital and 6 weeks of support to allow patients time to regain their independence and reduce re-admittance. This service is for Derby City residents only that have been referred by a health care professional.

When arriving home, all patients will receive a home check, and we will complete any essential cleaning jobs to get them settled.
Creating a support plan
After we have settled the patient home, we will carry out an assessment and create a support plan for them.
We may offer the following depending on their needs:
- Shopping online or in person
- Transport to medical appointments
- Wellbeing checks
- Befriending
- Introduction to community events
- Benefits advice & form filling
- Unpaid carer support
- Referrals to other agencies
